• For your first appointment, we ask that you arrive 5-10 minutes early to fill out paperwork.

• Please wear comfortable clothes that are appropriate to your injury. For example: shorts, sneakers, and t-shirts.

• Also, please remember to bring your insurance card, a form of identification, and your physical therapist prescription from your referring physician.
